Practice Model Crisis Intervention Case Study Jason was removed from his dysfunctional family at 10 and spent the next 4 years in residential care with another provider, where he maintained a cycle of violence and disruption.
As his overall behaviour significantly escalated his placement became increasingly disrupted. The placing authority required an urgent solution… This was Five Rivers high-impact integrated care model.
Jason was referred and admitted to one of our specialist therapeutic residential single bedded homes, which offered full 24 hour 2: Firm boundaries and an individual care plan tailored to his specific needs were established, underpinned by psychological knowledge and applied practice.
After 2 weeks Jason was returned to his original out of Five Rivers placement, despite our attempts to advocate that a continued more intensive period in solo placement was required to truly stabilise him. His local authority requested a 6 month placement in a solo residential home, as they felt it had had a successful impact during his last stay.
Jason was assessed, admitted, and a medium term bespoke care plan was developed as part of our therapeutic stabilisation and re-parenting programme.
Jason was also introduced to the local educational facilities through the pupil referral centre, where he responded well to his new environment, integrated and started to make new friends and accept the education being provided. Crisis Leads to Opportunities: After this time it was felt that he had achieved the required outcomes and as part of his overall pathway programme, he was ready to be moved to a group living home nearer his family.
This offered Jason further independence and potential reintegration with his family, and we fully supported and assisted in his move. We Still Keep In Touch: Jason is now 18 and as with many of our former Looked-after Children, we like to keep in touch.
